CT Storm Basketball Club

Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19

 The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. CT STORM AAU Girls Basketball has put in place preventative measures to reduce the spread of COVID-19; however, CT STORM AAU Girls Basketball cannot guarantee that any participants will not become infected with COVID-19. Further, attending CT STORM AAU Girls basketball events could increase your risk and your risk of contracting COVID-19.

By signing this agreement I affirm that I have not been diagnosed with, demonstrated any symptoms of or have in any way been exposed to any communicable diseases (including but not limited to the virus commonly referred to as COVID-19) within the past sixty days.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I/my child(ren) and I may be exposed to or infected by COVID-19 by attending CT STORM AAU Girls Basketball practice or tournaments and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at any CT STORM AAU Girls Basketball may result from the actions, omissions, or negligence of myself and others, including, but not limited to, CT STORM AAU Girls Basketball coaches, volunteers, and program participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself/my child(ren) (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with mine or my child(ren)’s attendance at, or participation in CT STORM AAU Girls Basketball (“Claims”). On my behalf, and/or on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless CT STORM AAU Girls Basketball, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the CT STORM AAU Girls Basketball agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any CT STORM AAU Girls basketball program.

 

Connecticut Storm COVID-19 Safety Plan

 

1. All coaches, players, attendees, and parents of CT STORM will sign a liability waiver prior to attendance in CT Storm basketball practices and tournaments this summer and fall.

  1. All coaches, players, attendees, and parents of CT STORM must conduct a daily symptom assessment.
  2. All coaches, players, attendees, and parents of CT STORM players must wear a mask when spectating, standing on the sidelines and while picking up or dropping off their athlete.
  3. Social distancing will be practiced when it is practical to do so.
  4. Basketballs and any other equipment will be sanitized before and after use
  5. Hand sanitizer will be available, and each coach, player, attendee, and parents of CT STORM will be required to use it.

I understand the above guidelines and agree that I/my child will adhere to these guidelines.  Failure to do so can results in either myself/my child to not be able to participate in CT STORM practices and or tournaments.
